Note on Names:
There are two names collected. If the same last name (1) and (2) is used by both entries, there will be one listing in the following format:
Last Name (1), First Name (1) and First Name (2) example: Corson, Bob and Mari
If two different Last Names are used then there will be two entries in the following format:
Last Name (1), First name (1) ex: Smith, John
Last Name (2), First name (2) ex: Doe, Jane

If you select N for Display address listing, then there will be no listing for that address.

You may also decline to be on the email list, but still be in the directory.
